The Norwalk Public Library is an official Passport Application Acceptance Facility U.S. 
Passport applications are accepted on a first come, first served basis.

PLEASE ALSO BRING

A Recent Passport Photo - 2x2
 Valid Driver's License, State Identification
 Card, Employee ID, or Military ID

Your Original Birth Certificate or Naturalization Certificate
(Certificate will be returned to address on your DS-11 application)

PASSPORT FEES

Check, or money order (No Cash) made out to the U.S. Department of State
will be accepted in the amount of
$110 for adults and $80 for children, and a $35 fee by
 check or money order
made out to Norwalk Public Library.

Norwalk Public Library is not accepting adult renewal applications. (form DS-82)
