Getting a Library Card for a Child Between the Ages of 5 and 17
|
Must be a Norwalk Resident:
Library Card Online Application Form for a Minor
- Parents Must Sign: Minors between the ages of 5 and 17 must have a parent or legal guardian present to sign the application.
- Internet Permission: A parent or guardian who wishes to allow a minor-age child to use the Internet without a parent or guardian present must sign the Internet permission box on the application form. A yellow Internet permission sticker is applied to the child's library card and the notation "I" is put in the child's registration information to signify parental permission was given.
Acceptable Identification:
To get a Norwalk Public Library System card for a minor, a parent or guardian
must present either one ID from section A or two ID's from section B.
A) The following IDs are acceptable by themselves, if
they are signed, valid (unexpired), current:

US passport with home address. (Some passports do not list
address in which case a second ID is needed.)

B) Two IDs required: One from each column.

Out-of-Town Users:
First-time users of the Norwalk Public Library must present their hometown library card and a parent or guardian must be present with an acceptable form of identification.
REPLACEMENT FEE FOR A LOST LIBRARY CARD IS $1.00.
